University policy requires that enrollment materials be submitted by the end of the month prior to the pay period you would like to begin making contributions. For instance, forms for the January pay period(s) must be received by the end of December.
The step-by-step instructions are as follows:

Complete the
Lincoln Alliance® enrollment form (see forms below)

Complete the UGA Salary Reduction Agreement (see forms below)

Mail all documents to Jeff Young at Cannon Financial Strategists, Inc. at the address on the bottom of the enrollment form or fax them to his attention at 706-548-4526
Jeff will contact you to confirm receipt of the forms. He will submit the forms for processing to UGA and Lincoln.
